REPORT DATE/TIMES/LODGING

You are to report to The NEW Villages High School Football Stadium (2210 Dr. Randy McDaniel Way, Middleton, FL) at 3:30pm-5:00pm on Sunday, December 11th . After Check In, dinner and practice, you will be bused to the Home2 Suites (7509 Penrose Place, Wildwood….352-913-2100) for your stay while at the game. We will bus players to and from practice while at the game. The first official meal will be dinner when you arrive and have a night practice after dinner. If there is a problem with the reporting time, please let us know. Food, lodging, insurance and transportation for your participation in the Game will be paid for through the All-Star Game budget. Your coach or parents are responsible for getting you to The Villages and home following the Game. Any Player that drives to the game will be required to turn in their keys to the coaching staffs and will get them back on game day. We will transport them while they are at the game.

ITEMS TO BRING

- Copy of Physical and Medical History From (ask your coach/school)
- Copy of Insurance Card
- Helmet, Shoulder Pads, Girdle w/pads, practice shoes, game shoes, etc
- Any other pads/braces/etc that you use for practice/game
- Extra practice shorts/T-shirts
- Extra Helmet decals to swap with teammates
- Inside shoes in case of inclement weather
- Casual Clothes, Slacks/Jeans, shirt for Banquet
- A Great Attitude and Competitive Spirt!

A practice jersey and shorts/T-shirt will be provided when you get to The Villages along with your Game Uniform. You will get to keep your Game Jersey and will receive a special Player’s Package (warm-up, t-shirts, backpack, awards etc.)

Tickets for the game can be purchased at the gate for $10 each. The game will be live streamed at 1pm on December 14th . A link will be shared later.

ALL STAR BANQUET

We will have an All-Star Banquet on Tuesday, December 13th at The Villages HS Cafeteria at 7pm. Any parents/coach wishing to attend, needs to download, complete and send us the form. Please mail form and check/cash/money order to the address listed on form.

ALL STAR RING

You have the option to order an All-Star Ring for $185. We have a “Ring Store” link set up with ZOLNIER Champion Rings where you order, pay a deposit or pay in full for the ring at that link. That link and or a QR Code will be given to every player at check in on December 11th. The deadline to order rings will be January 10, 2025. Zolnier will send your ring direct once the balance is paid in full. We will be able size your ring at the event and delivery will be March 2025.

Parent Consent

I hereby approve my child to participate in the FACA All-Star Classic, practice and related activities. My child has no medical or emotional problems which may affect his/her ability to safely participate in your program.

Regarding routine first aid, major emergencies or medical trauma, I understand that the All-Star Team Doctors and staff would provide whatever care or treatment they reasonably could and would refer to the appropriate physician the further treatment of such. I hereby authorize consent to any X-ray, examination, anesthetic, medical or surgical diagnosis or treatment or hospital care, which is deemed needed and rendered under the guidance or special supervision of the physician.

Being fully aware of the hazards and possible consequences involved in treatment of the above described routine and major emergency conditions, I being legally competent to give consent, hereby consent to such treatment and agree to hold the Florida Athletic Coaches Association, the Organizers, Sponsors and Supervisors and/or all of them, free and harmless from any claims, whatever which may result from such treatment.

All medical expenses incurred due to my child's participation in the FACA All-Star Classic, practice and activities are understood to be the responsibility of the participants Insurance Carrier with the All-Star Insurance Carrier being an Excess policy (primary if participant has no coverage) and I hereby give authorization to provide such necessary insurance information to be used should my child incur an injury or illness that requires medical attention.
